If you are up to date with your physical (having had a physical within the last year) and need a form completed, we can easily do that for you. You will need to complete a form called 'Interim Medical History Questionnaire'. This form will let us know what has happened since your physical; perhaps you had surgery or saw a specialist. You can find the Interim form on our website in the 'Forms to Complete' section under the 'Forms' tab. You can then drop the Interim form and the form you want us to complete in the 'Forms' box inside the door of one of the offices. We request you give us 5 business days for completion, and there is a $7.00 fee since it is not done at the time of the physical. You can either pay when you drop the form off or when you pick it up. Unfortunately email is not HIPAA compliant and we cannot accept patient information via email. If you have any questions please do not hesitate to call our office.
